Steps to Apply for Birth Certificate Issuance:

Basic Information:

  • Select Primary Language: Choose your preferred language from the dropdown list.
  • Select Secondary Language: Choose the secondary language as needed.

Birth Information of the Child:

  • Type of Service: Select whether you are applying for birth registration or certificate issuance.
  • Place of Birth: Choose the place where the birth occurred (e.g., Home, Hospital).
  • Child’s Name: Enter the full name of the child, including First Name, Middle Name, and Last Name.
  • Child’s Sex: Select the gender of the child from the dropdown list.
  • Date of Birth: Enter the date of birth in the format ( DD/MM/YYYY ).
  • Weight at Birth: Provide the child’s birth weight.
  • Aadhar Number (if available): Enter the child’s Aadhar number, if available.
  • Registration Number of Previous Birth (if applicable): Enter this if the child had a previous registration.

Address:

  • Current Address: Select whether this is a permanent or current address.
  • House Number: Enter the house number or locality details.
  • Street: Enter the street or road name.
  • City/Village: Provide the name of the city or village.
  • District/State: Choose the district and state from the dropdown menus.
  • Pin Code: Enter the postal pin code.

Mother’s Information:

  • First Name/Middle Name/Last Name: Enter the full name of the mother.
  • Aadhar Number: Provide the mother’s Aadhar number, if available.
  • Mobile Number: Enter the mother’s contact number.
  • Email ID: Provide the mother’s email address.

Father’s Information:

  • First Name/Middle Name/Last Name: Enter the full name of the father.
  • Aadhar Number: Provide the father’s Aadhar number.
  • Mobile Number: Enter the father’s mobile number.
  • Email ID: Provide the father’s email address.

Address of Parents During Time of Child’s Birth:

  • House Number/Street: Enter the house and street details where the parents resided at the time of the child’s birth.
  • City/Village: Provide the name of the city or village.
  • District/State/Pin Code: Select the district and state from the dropdowns and enter the pin code.

Informant Information:

  • First Name/Middle Name/Last Name: Enter the full name of the informant (person applying).
  • Aadhar Number: Provide the informant’s Aadhar number.
  • Relationship to Child: Select the relationship to the child (e.g., Parent).
  • Mobile Number: Enter the informant’s contact number.
  • Email ID: Provide the informant’s email address.

Type of Village/Town Where Child Was Born:

  • Select City: Choose the city where the child was born.
  • Type of Area: Specify whether the area is rural or urban.

Educational Qualification of Mother and Father:

  • Education of Mother: Select the educational level of the mother from the dropdown.
  • Education of Father: Select the educational level of the father from the dropdown.

Family Religion:

  • Religion: Choose the family’s religion from the dropdown.

Occupational Information of Parents:

  • Occupation of Mother: Enter or select the occupation of the mother.
  • Occupation of Father: Enter or select the occupation of the father.

Child’s Information at the Time of Birth:

  • Weight of Child: Enter the child’s weight at birth.
  • Child Born in Maternity Ward/Other Location: Mention whether the child was born in a hospital, maternity home, or another location.

Sign Off & Declaration:

  • Signature: Enter your name to confirm the declaration.
  • Check the Box: Agree to the terms and conditions by checking the box.

Submit:

After completing all sections, click the “Submit” button to finalize the registration or application.

Additional Information:

  • Track Application Status: After applying, you can track its status by clicking the “Track application status” option in the navigation menu.
  • Important Notes: Make sure all details provided are accurate to avoid delays in processing. The service does not follow a “First In, First Out” model, and there is no deadline for submission.
